# CRH, ACTH and Cortisol: How the Long-Term Stress Axis Works

The long-term stress response is often explained through a chain involving CRH, ACTH and glucocorticoid release from the adrenal cortex.

The body’s longer-term stress response is often taught as a chain of signals rather than a single event. In that chain, CRH, ACTH and glucocorticoids each play a distinct role.

This sequence matters because it shows how a stress signal can move from the brain into the endocrine system and then influence metabolism through the adrenal cortex.

## The first step: stress reaches the hypothalamus

In this teaching pathway, a stress signal activates the hypothalamus. More specifically, the paraventricular nucleus is then involved in releasing corticotropin-releasing hormone, or CRH.

CRH is the first major endocrine messenger in this sequence. Its role is to carry the stress signal forward to the pituitary gland.

## The second step: CRH stimulates the anterior pituitary

CRH travels through the hypothalamic-hypophyseal portal system to the anterior pituitary. There, it acts on corticotroph cells.

These corticotrophs then release ACTH. That release is part of a receptor-driven signalling process that, in this explanation, involves cyclic AMP and protein kinase activity before ACTH enters the bloodstream.

## The third step: ACTH reaches the adrenal cortex

Once ACTH is in circulation, it travels to the adrenal glands above the kidneys. Its main role in this overview is to influence the adrenal cortex, the outer region of the gland.

This is where the stress signal becomes linked to hormone production that supports the body during ongoing stress.

## The cortisol link

The zona fasciculata of the adrenal cortex is described here as the main glucocorticoid-producing region. It releases hormones such as cortisol, cortisone and corticosterone.

In physiology teaching, cortisol is often the name people remember most. It is associated with the metabolic side of stress, especially the handling of glucose so the body has usable fuel during prolonged challenge.

## Why this is different from fight or flight

The long-term stress axis should not be confused with the immediate fight-or-flight response. The adrenal medulla handles that faster response and releases catecholamines such as epinephrine and norepinephrine.

That system is more closely tied to the nervous system. The CRH-ACTH pathway, by contrast, is part of the slower endocrine response to ongoing stress.

## Why the pathway matters

This sequence is useful because it makes stress physiology easier to follow. Rather than seeing ACTH as an isolated hormone, it becomes part of a clear progression:

### Stress signal

A stressor activates centres linked to the hypothalamus.

### CRH release

The hypothalamus releases corticotropin-releasing hormone.

### ACTH release

The anterior pituitary responds by releasing ACTH.

### Adrenal cortex response

The adrenal cortex produces hormones that help the body adapt metabolically.

This structure is one of the reasons ACTH remains such a common teaching topic in endocrine physiology.

## Final thoughts

CRH, ACTH and cortisol-related signalling form a clear teaching model for the body’s longer-term stress response. Together, they show how the brain, pituitary and adrenal cortex work as a coordinated axis.

For readers exploring peptides uk as part of wider endocrine education, it is worth understanding the pathway before focusing on products. Many people search where to buy peptides or plan to buy peptides uk before they understand the biology itself.
